The Care Card

A one-page introduction for whoever’s looking after someone for a few days — respite care, a new aide, a hospital stay, a babysitter covering for you. Warmer than a medical form, still covers what matters.

Add a photo, a nickname, the things they might forget to mention (pain, communication needs, what helps), allergies, medications grouped by time of day, doctor info, and emergency contacts. There’s room for a note at the end too — text, or an image, so you can drop in a QR code, a link, or whatever feels right.

Your data stays on your device. Nothing you type or upload — including any photo — is sent anywhere. It lives in whichever browser you fill it out in, so use the same device each time.
